Our Mission

At All Seasons Health, our mission is to provide compassionate, comprehensive, and personalized in-home healthcare that supports individuals and their families through every stage of aging and illness. We understand that home is more than a place, it’s where people feel safest, most comfortable, and most themselves.

That’s why we’re committed to providing our patients with services that are not only clinically excellent but rooted in empathy. Whether it’s managing chronic conditions, navigating complex diagnoses, or providing comfort at the end of life, our team delivers primary, palliative, and hospice care that respects the dignity, wishes, and unique goals of each individual we serve.

Our Services

All Seasons Health offers a full range of in-home medical services tailored to meet the evolving needs of older adults. From preventive care and chronic condition management to palliative support and end-of-life care, our team provides consistent, thoughtful attention at every step. We prioritize open communication, personalized treatment plans, and a collaborative approach that keeps patients comfortable, informed, and supported, whether they're facing day-to-day health challenges or more complex decisions about long-term care.

Casey Leno, MDiv

Casey is a Board Certified Hospital Chaplain with a Masters degree in Theology, Certified Grief Counselor, and a Black Belt in Lean Six Sigma. He began his career as a Hospital Chaplain and soon moved into director roles in Hospital Administration leading out in Patient Experience through Mission, Ministry and Community Health Integration

Casey is a Board Certified Hospital Chaplain with a Masters degree in Theology, Certified Grief Counselor, and a Black Belt in Lean Six Sigma. He began his career as a Hospital Chaplain and soon moved into director roles in Hospital Administration leading out in Patient Experience through Mission, Ministry and Community Health Integration. Casey used his experience and education to help start ThriveCare Assisted Living, focused on creating a home-like environment where seniors are supported and cared for through life enriching integration. Casey’s goal is for every resident and patient to truly feel supported and cared for in their individual health journey.

Erika Leno, RN

Care. Erika's background in direct patient care,  along with her experience in owning ThriveCare Assisted Living, inspired her to create a better continuum of care for our elderly population. She co-founded All Seasons Health to provide that continuum through in-home Primary, Palliative and Hospice Care. Erika has a passion to be a part of patients' health journeys while supporting their goals of care as they age in place

Alyson is a Board Certified Nurse Practitioner with over 20 years of experience caring for individuals with chronic and advanced illness. Her expertise is in primary care of the older adult, as well as hospice and palliative medicine. She completed her BSN, MSN and Doctorate in Nursing Practice at the University of Colorado. Alyson is a Board Certified Nurse Practitioner with over 20 years of experience caring for individuals with chronic and advanced illness. Her expertise is in primary care of the older adult, as well as hospice and palliative medicine. She completed her BSN, MSN and Doctorate in Nursing Practice at the University of Colorado. During her time working with the older adult population, Alyson noticed that the current healthcare system doesn't always meet the needs of our growing senior population. Determined to do something about it, she teamed up with Barb, Casey and Erika to launch a model of care that provides our older adults with the time, convenience and support they deserve.
